This office is your home base for health care. Our team knows you, keeps your full health picture in one place, and helps you get the right care at the right time, whether that is here, with a specialist, or after hours.
 

We are partners in your health, through every Phase of your life.

Our Commitment to You

  • Listen to your concerns, questions, and goals – they matter.

  • Explain your diagnoses and treatment in plain language.

  • Hold same-day appointments for acute concerns. Call us first, even for urgent things, before going to an ER or urgent care, unless it is a life-threatening emergency (then call 911).

  • Be reachable 24 hours a day – after hours, call the office and press 1 for any urgent concerns.

  • Tell you your test results, usually within 1 to 2 days. If you have not heard within 5 business days, please call us.

  • Coordinate your referrals, give you the specialist's name and reason, and follow up on the report.

  • Follow up on your test results, referrals, and care plans.

  • Keep your health information private.

Your Role in Your Health

  • Be honest and complete when you share your history, symptoms, and concerns.

  • Ask questions if anything is unclear. There are no bad questions.

  • Call us first with health problems, unless it is a life-threatening emergency (then call 911).

  • Follow the care plan we agree on, or tell us if something is not working.

  • Keep your appointments. If you cannot make it, please give us 24 hours' notice.

  • Bring your medicines, or a current list, to every visit.

  • Tell us about care you get elsewhere, and ask those providers to send us a report.

  • Treat our staff with respect – they are here to help you.

Where Should I Go for Care?

Colds, flu, fevers, ear aches, sore throat, rashes, minor cuts and burns, sprains, UTIs, medication questions, anything you are not sure about.

Call 911 or go to the ER

Chest pain, trouble breathing, signs of stroke (face drooping, arm weakness, slurred speech), severe bleeding, head injury with loss of consciousness, seizures.
