Before You Arrive

Filling these out at home saves time and lets Dr. Christofis focus on you rather than forms during your visit. You will be asked about your medical history, current medications, allergies, family history, and any concerns you want to discuss.

Please bring the following with you:

When You Arrive

This may include checking your blood pressure, heart and lung sounds, reflexes, and other basic measurements. If lab work is needed, we can often draw blood right in the office and have results back quickly.
