Here is everything you need to know about your new patient visit.

What to Bring to Your First Appointment

Being prepared for your first visit helps us make the most of your time together. Please bring the following items to your appointment:

  • Photo ID: A valid driver's license, state ID, or passport
  • Insurance card: Both front and back, including any secondary insurance
  • Medication list: All current medications, vitamins, and supplements with dosages
  • Medical records: Any recent lab results, imaging reports, or specialist notes from your previous doctor
  • Immunization records: If available, especially for new patients establishing care
  • List of questions or concerns: Anything you want to discuss during your visit

If you do not have all of these items, do not worry. We can still see you and work on gathering records over time. The most important thing is that you show up.

How Long Does the Visit Take?

Plan to spend about 45 minutes to one hour at our office for your new patient visit. This includes time for check-in, paperwork, and your appointment with your physician. We set aside extra time for new patients because we believe that a thorough first visit sets the foundation for your care going forward.

We recommend arriving 15 minutes early to complete any intake forms and allow time for insurance verification. You will also receive a text and email ahead of your appointment with the option to complete these forms online.

What Your Physician Will Cover

Your new patient visit is about more than just a physical exam. It is an opportunity for your physician to understand you as a whole person. During the appointment, your physician will:

  • Review your complete medical history, including past surgeries, hospitalizations, and chronic conditions
  • Discuss your family medical history to identify any inherited risk factors
  • Go over your current medications and make sure everything is up to date
  • Perform a physical examination appropriate to your age and health needs
  • Talk about your lifestyle, including diet, exercise, sleep, and stress
  • Address any current symptoms or health concerns you may have
  • Discuss preventive screenings and vaccinations you may be due for
  • Create an initial care plan and, if needed, order lab work or referrals

This visit is also your chance to ask questions and share what matters to you about your healthcare. We encourage open, honest conversation so we can provide the best care possible.

Transferring Records from Your Previous Doctor

Having your medical records helps your physician understand your full health picture. There are a few ways to transfer your records to Phase Medical Group:

  • Call your previous doctor's office and request that they fax or electronically send your records to our office
  • Sign a medical records release form, which we can provide at your first visit
  • Bring copies of recent records, lab results, or imaging reports with you

If your records are not available before your first appointment, that is perfectly fine. We can begin your care and request records to arrive in the following weeks. 

Insurance and Payment

Phase Medical Group accepts most major insurance plans. We recommend calling our office before your visit to confirm that we accept your specific plan, or you can check our current list of accepted insurances by clicking here.
